    For those areas of the operation that are being converted to non-forest use, please complete the attached 'LUC Reforestation Exemption' form, collect the required signatures, and return the completed & signed form to the local ODF office along with a detailed map of the area being converted. The completion of this form will exempt the Landowner from the reforestation requirements of the Forest Practices Act for the portion of the operation being converted. All other areas of the operation not being converted will still be subject to the Reforestation Rules as described in the Forest Practices Act.
