I met onsite 1/24/2022 with the landowner and Michael, an ODF Geotechnical Specialist, to view and evaluate a slide that according to the landowner occurred on January 6th from this unit. It appears that the slide started in the buffer above the house and became a debris flow down the draw and emptied out onto the driveway and around the house. No one was injured. The landowner has replaced the plugged culvert under the driveway. The Geotech recommended that the slash piles on the top edge above the slide be moved. I have contacted Steve Garner and informed him of this request. There is additional soil material perched within the slide area, and will likely slide down in the future.

No change from the last inspection, except that the equipment mentioned has been removed. There has been no communication with ODF from the operator, notifier, or landowner regarding ODF's concerns. Enforcement action will be initiated.

Logging operations have terminated. Shovel and skidder still on site and staged for removal. The haul road needs to shaped, ditches cleaned, and culvert inlets cleaned out, all to facilitate proper drainage. The draw above the blue house has not been cleaned out as requested in the last inspection. The last area to be logged, below the house at the top of the unit, has a skid road that needs to be water barred, and this is the only area remaining that needs to be planted.

The 4/9/19 Inspection addresses the amended Written Plan. The stream channel above the blue house should have the logging debris cleaned out of it so drainage is not hampered, and debris is not delivered to the blue house property .

ODF geotechnical specialist Mike Buren has reviewed the Written Plan and states that it appears to meet or exceed the Div. 623 rule requirements.
