Resource review: 1) Sugar Pine Creek is LF 2) Ensure no operations within 100ft of LF without a reviewed written plan on file 3) All other streams SN 4) No instream work to include crossings without ODF approval of low water conditions 5) The operation has steep or erosion-prone slopes or high landslide hazard locations. No ground-based machinery activity may occur on slopes which are steep or erosion-prone within 100 feet of any stream or on high landslide hazard locations, without a State Forester approved Plan for Alternate Practice 6) No other resource conflicts 7) Two leave trees and 20 ft.³ of down wood is required per acre 8) Avoid hauling or spreading rock on extreme wet weather to avoid wet weather road use concerns regarding delivering sediment into Waters of the State 9) Ensure sediments do not enter waters of the state 10) Comply with all state and federal rules and regulations and follow herbicide label instructions 11) Must meet all ODF fire prevention and smoke management requirements 12) Ensure compliance with the rules of Oregon's "Smoke Management Plan" 13) Obtain a burn permit from ODF Medford office prior to burning 14) Waiver of the 15-day waiting period as requested has been granted
